Equipment Handout for
Football &
Cheer uniform
measuring for Cheer
at the Football Field on Saturday, June 25th
from 8:30am – 10:30am
Anyone who has already registered but has outstanding
paperwork please drop it off this day and football players
should attend to pick up equipment, as well as all
cheerleaders should attend to get measured for uniforms:
Original Birth Certificate – Raised Seal
is required by Pop Warner
only for NEW participants
Release Form - signed by both parents/guardians
Equipment/Uniform Deposit - check for $200 dated
12/1/11
This check will be returned to you at the end
of the season when
all equipment/uniforms are returned
Medical Release form - dated after 1/1/11-MUST submit
before August 1
End of Year Report Card - June 2011-MUST submit before
August 1

Flag Football & Tiny Mite Cheerleading:
(AGES 5 & 6):
Cost $50 per participant
Cheerleading: (AGES 7+): Cost $125 per participant
(Cost includes Briefs, Bow and 2 pairs of socks)
Tackle Football, (AGES 7+): Cost $125 per participant
2nd Child - $100.00 -- Family Cap - $300.00
